Additional Dental School Forms with Instructions on Completion

Dear Dental School Student, Faculty or Staff

To maintain the highest possible quality of patient care it is essential that there is complete and accurate documentation of all patient-related care in the patient record. This must include the following:

  1. An accurate treatment plan that is signed by the patient and provider(s)
    1. If there are modifications to existing treatment plans, such changes must be clearly documented and signed by the patient
  2. A signed informed consent for all Urgent Care, OMS procedures, implant placement.
    1. Patients should be seated in an upright position and must have all proposed procedures explained to them
    2. Patients should be afforded the time to ask questions
    3. Patients must understand what treatment has been proposed
      1. If patients do not speak English, the PCCs have a list of multilingual providers who may be called upon to assist in this process
  3. Any and all research protocols must have an IRB approved consent form and these must be signed by each and every subject in the study.
    1. All IRB requirements must be followed exactly as stated in the IRB approval.
  4. All procedures and drugs/medications used during each and every appointment must be clearly entered into the progress notes of the dental chart and must be signed by the student and by the faculty covering the procedure.

When you have been unable to reach a patient, a yellow card is sent to attempt to make contact.

If no response is received or the patient wishes to be inactivated, the letter (next page) is sent to inform him/her of inactivation.
