Dental School Medical Emergency Response Protocol

When assistance is needed (after no response from a verbal outcry), leave to get help after reassuring the victim that you will return as quickly as possible:

During the Normal Business Hours of Monday through Friday 8:00 AM to 5:00 PM; Tuesday, Wednesday and Thursday (C-3 Clinic) 5:00PM to 7:00 PM:

  • For Immediate Life-Threatening Emergencies, or Serious Behavioral Incidents:
    • Use a clinic phone to call Campus Police by dialing 711 (DO NOT dial 9 first). Campus Police will call for an ambulance if needed, and/or send officers to assist.
    • Follow a call to campus police with an immediate page for the Dental School Emergency Response Team
  • For Non-Life-Threatening Emergencies:
    • Page the Dental School Emergency Response Team only
  • For A Glucometer Test or Blood Pressure Verification on an Asymptomatic Patient:
    • Members of the Emergency response team can be paged individually at the pager numbers listed at the bottom of this page. Nurses are to be paged before the Doctor

After the Normal Business Hours or when there is no nurse coverage or no nurse available:

Use clinic phones to call 711 (campus police will call for an ambulance or come to assist as needed). Report the incident to a Dental School nurse verbally or by email at the earliest opportunity.

Protocol for Dental School Emergency Response Team Paging:

The Emergency Response pager number is programmed into the speed dial location on clinic wall phone and is marked as “Emergency” and highlighted in pink. From a non-clinic phone within the building, page using the number 9-410-389-1324. After triple beep tone, enter the nearest room/quad # where the emergency is occurring

  • Exceptions: ground floor -enter letter G as zero: G205 would be 0205
  • CMS on lower level -enter 777

Emergency responders will report to the room/quad & provide treatment.

Before Responders Arrive:

  • If bodily fluids present and/or victim is coughing, don the appropriate PPE
  • Position victim so as to protect from further injury on the floor or in a dental chair.
    • If patient is feeling faint or has fainted, lower head of chair and/or raise feet unless doing so might cause further injury.
  • If campus police have been called to send an ambulance, designate a person to alert the guard in the atrium lobby, and to assist the EMTs to the emergency.
  • If possible, bring or have someone bring the Emergency Cart/Oxygen tank into the room/quad (located outside clinic prep area or in some side hallways off main corridors).
    • Start oxygen with a nasal cannula for oxygen support at 2-4 L/min, or with a non-re-breather mask, if the patient is distressed due to shortness of breath, at 10-15L/min.
    • Provide treatment if trained; at minimum assess ABCs (Airway, Breathing, and Circulation)
    • Start CPR if necessary for the response team!

Exposure Reporting and Management Procedure: Baltimore Campus

If an exposure has or may have occurred:

  1. STOP and IMMEDIATELY remove item responsible for exposure to prevent a DOUBLE exposure
    • Note if item is visibly bloody
  2. Remove PPE, set aside gloves for checking later if a glove breach is not obvious (SOD nurse can help)
  3. Wash all wounds with soap and water, and flush exposed mucous membranes with cool water
    • Do not force wounds to bleed
  4. DO NOT dismiss source patient/stop patient from leaving (if possible), if discharge has occurred
  5. Prepare to report the incident, make note of patient medical history/risk factors and have patient ID #
  6. Report incident to your attending faculty (student/resident), or department supervisor (staff)
  7. Report exposure to an SOD nurse; the most efficient way to do so is to use the emergency pager system
    • To place an emergency page:
      • Locate the button marked Emergency on a clinic wall phone and press (emergency pager number will be auto dialed) or dial 410-389-1324 (dial 9 first if using an SOD phone)
      • After 3 beeping tones are heard, enter your room or quad number, followed by the # sign (lower level dial 777)
      • Nurse(s) will respond to your area within a few minutes (page again, if no response in 3-5 minutes)*
    • If unable to contact SOD nurse, phone Student and Employee Health/UMIC (410-328-1362) for advice on post exposure follow-up, indicate an exposure has occurred and ask to speak directly to Dr. Baronas or Denise Mitchell, PA. After 5pm, Dr. Baronas can be reached by cell phone at 410-608-0337 (DO NOT go to the ER without a referral from Dr. Baronas)
  8. Complete the required forms (all incident details must be provided to an SOD nurse as soon as possible)

Medical Emergency Policy CL006

  1. ALWAYS call for help before doing anything else!

    If help does not arrive in response to a verbal outcry, reassure the victim that you will return as quickly as possible and go to get help!

  2. Retrieve or send someone to retrieve the nearest emergency cart.
  3. Position the victim to protect him or her from injury (dental chair or floor).

    Supine position is best except for chest pain or respiratory distress/difficulty breathing.

How do you get Help for the Conscious Victim

Call the Dental School Emergency Response Team

  • Press highlighted “Emergency” speed dial button
    • Or: Call 9-410-389-1324 (Emergency Response Team pager)
    • Or: if using a cell phone, same number minus the 9
  • Wait for 3 beeps
  • Enter nearest room/quad # (use the zero key to indicate Ground Floor)
    • G202=0202

    Enter room location carefully! If the SOD, ER Team can’t find you, they can’t help.

  • Hang up, and return to assist with the emergency
    • If possible, have someone be on the lookout for the Emergency Response Team, to assist in guiding them to the victim
    • Reposition if necessary to prevent further injury
    • Ask nearest person to bring to the injured person the Emergency Cart/Oxygen tank for oxygen support and/or be prepared to get AED and begin basic CPR if needed.
    • start oxygen with a nasal cannula at 2-4 L/min for oxygen support, or use a non-rebreather mask if patient is distressed due to shortness of breath, set between 10-15 L/min (make sure bag is inflated)
    • If patient is unresponsive to change in position (lowering head and/or raising feet, tapping shoulder and verbal, “are you ok?” , begin chest compressions and attaché pads of the AED to the chest as soon as possible
    • Repeat if necessary
  • Repeat the page if no one arrives in 5 minutes, and defer to attending faculty for guidance
    • If no response after second page, an SOD nurse is most likely unavailable
    • Report details to an SOD nurse as soon as possible

How do you get Help for the Unconscious Victim

  1. Call Campus Police Emergency Line (711) from campus phone
    • Send someone to the 1st floor lobby to alert the officer and escort emergency personnel (EMS) when they arrive
  2. Call Dental School Emergency Response TeamCampus phone

Wheelchairs must be checked out at front desk locations (patient’s must leave a valid ID until chair is returned). Wheelchairs checked out from a nurse’s office, must be checked out from the nurse, and returned to the same location ASAP.

How do you get Help for Other (Non-Medical) Emergencies

Call Campus Police Emergency Line (711) from a campus phone, and provide the nature and location of the emgergency.

  • Campus Police will call 911, and send someone to help

Fire and Fire Evacuations

  • If a fire occurs, pull a red fire pull alarm (located throughout the building)
  • Evacuate the building using the stairwells- walk calmly, no pushing or shoving!
  • If unable to take the stairs –go to area of rescue assistance at the bank of three elevators used for staff/students on each floor in the front region of the building
    • Have a Dental School Fire Warden accompany you
    • Another Fire Warden should be told that you and/or patients are in the area of rescue assistance
  • If you are unable to leave a patient under sedation, let a Fire Warden know and they will alert the Fire Marshal/Fire Department when they check in on the ground floor
  • All Dental School Fire Wardens must check in on the ground floor by the guard’s desk
    • If someone is still in the building, it must be noted on the sign in sheet at the guard’s desk
  • Once out of the building cross the street and stay away from the building until an “all clear” is announced.

Do not put yourself, your patient, or others at risk by trying to exit via a stairway

Additional Procedures for Hygiene Department at the Baltimore Campus

Fire drill procedures (student)

  1. Turn off main power switch to chair and cover instruments with patient bib.
  2. Evacuate patients (including their valuables) following Dental school protocol.
  3. Stay with your patient
  4. SODs will help obtain wheelchairs for non-ambulatory patients and coordinate with the fire warden and faculty.
  5. If you are on “block assignment” in the school, please make every attempt to make contact with a hygiene faculty member once outside of the building. This is particularly important if this is a true emergency.
  6. Once the building has been cleared for return, escort your patient back to the treatment area and resume your procedure.
  7. Document the fire drill and the amount of time away from the clinic in your progress notes.

Fire Drill procedures (faculty)

  1. Ensure all of your assigned students have properly evacuated themselves and their patients. Students on blocks will be under the supervision of the faculty of that department but should be accounted for.
  2. Advise fire warden of any (and #) students with non-ambulatory patients.
  3. Evacuate yourself.
  4. Once outside, check with other faculty members about need for additional assistance and begin documenting the hygiene students that have cleared the building.
  5. If this is a true emergency, we will need to provide a list of possible persons in the building and where they might be.

WEATHER ALERTS: The Dental School weather alert hotline is 706-3368 (706-DENT). Please call this number during inclement weather for information related to school/campus closings, clinic closings, and class cancellations. NOTE: There are times when, because of the weather, the clinics may have to close for patients while the school is otherwise open for staff.

In addition, UMB maintains a campus emergency number and Web site. The Campus Emergency Information Phone Line is 410-706-8622. The Weather Alerts page http://www.umaryland.edu/alerts/index.html#weather.

BUILDING EMERGENCIES: If you are in the building outside of normal work/school hours and experience an emergency related to the facility (e.g., power outage, equipment failure, flooding, etc.), you may call 410-808-8120 to reach the facilities director (Kate McManus). Initially, if guards are posted, please advise the security personnel of the emergency.

PATIENT EMERGENCIES: For patients of record, the emergency number for the school during the holidays is 443-827-5011. We have a coordinated system in place with the oral surgery and emergency departments at UMMS to help facilitate emergencies.
