Add new Visit Form for each visit
Click on Forms go to Visit form
Highlight Visit Form and click
Then click “ok” new form will appear
Creating Visit Form
Hit the calculate button and patient information will appear as provided in medical history. Complete ALL lines that pertain to the patient treatment done for the session. Hit calculate so information is added to the form. REMEMBER to hit calculate before having visit form approved by faculty.
Deleting Unapproved Note

Clinical data will be entered in 3 additional tabs of the newly named ‘Medical Hx and Exams’ form (MDHX). See new tabs below.
- 3rd Tab – Occlusal Exam including full TMD
- 4th Tab – Radiographic Interpretation (Image evaluation must be approved inMiPacs)
- 5th Tab – Periodontal Exam
‘Student Worksheet’ (STUWRK) form will collect student worksheet data such as Problem Lists, Diagnoses, Prognoses, draft Treatment Plans, and Recommended Home Care. Only faculty and students will have access to this form as you draft, revise and gain approval during your treatment planning work.
